Costs

How much does rehab cost with NYSHIP?

With an Empire Plan network program, your cost is mostly copayments — inpatient stays are described as paid in full and outpatient visits may carry a $25 copay — up to an annual in-network out-of-pocket limit. Non-network care can cost far more, because a deductible, coinsurance and charges above the allowed amount may all apply.

How do you estimate your rehab cost in five steps?

  1. Confirm your guide. PE and C-82 have different in-network limits. See which members each guide covers.
  2. Confirm network status for the exact level of care — see how to check whether a rehab is in network.
  3. Confirm certification — uncertified care may receive no benefits.
  4. Pick the right formula: network copays; non-network inpatient 10% of billed charges; non-network outpatient deductible then 20% of allowed amount.
  5. Add balance-billing risk for non-network care, and check where you stand against annual limits.

Network status follows the Empire Plan's own hospital and provider contracts, which are not the same as Empire BCBS (Anthem) commercial networks. See Empire Plan vs Empire BCBS (Anthem) before assuming a hospital is in network.

What might different scenarios cost?

Network inpatient stay, certifiedApproved stay paid in fullAny non-covered items; separate copays elsewhere
Network IOP, 30 billed visitsUp to 30 × $25 = $750 if each visit carries a copayHow the program bills each day
Non-network inpatient, $40,000 billed10% = $4,000, but coinsurance stops at the $3,750 combined maximumPractitioner bills, uncertified days, items not counted
Non-network outpatient, 10 sessions$1,250 deductible, then 20% of allowed amount + any balance billGap between bill and allowed amount

Which annual limits can stop your costs?

Network costs are capped by the in-network out-of-pocket limit for Hospital, Medical/Surgical and MHSU expenses: $2,750 individual / $5,510 family under PE, and $2,670 / $5,350 under C-82 (printed p. 2). Non-network coinsurance is capped by the combined annual coinsurance maximum of $3,750 per enrollee, spouse/partner or all dependent children combined (printed p. 3) — but that cap does not cover everything. What costs do people forget to budget for?

  • Prescription copays, which fall under the separate Prescription Drug Program limit
  • Travel, childcare and time away from work — see medical leave for rehab
  • Optional amenity fees at private programs — see luxury rehab
  • Aftercare such as outpatient therapy following residential treatment

Ask the provider for a written estimate that shows

  • Each service you expect to receive and its billing code if available
  • Which charges they expect the plan to pay
  • What they will bill you, including any amount above plan payment
  • Optional services you can decline

Frequently asked questions

Is rehab free with NYSHIP?

Not necessarily. Approved network inpatient stays are described as paid in full, but outpatient copays, prescriptions, non-network costs and uncovered items can still apply.
